These forms include the tenure/promotion dossier cover sheet, which provides a useful summary of data concerning the candidate and a summary of tenure/promotion recommendations made; it also includes the recommendations of the dean and chair. See Section 3.8 (tenure) and 3.9 (promotion) of the HSC Faculty Handbook.
